[FIXED] Installed TTW + a few UI mods but Immediately experiencing bad hitching and 1% lows of 20fps

Hey everyone,
As it's been a good number of years since I last played the game and/or touched Bethesda modding I've been following a mod tutorial by Gopher.
Although it's been quite a few years I am experienced with modding so I know there are dos and don'ts, and I do follow steps exactly, sanity check, and test thoroughly after each install.

So far I haven't even gotten to texture packs or lighting mods yet, all I've done is installed a few UI mods and essentials like the 4GB patcher, and yet I'm noticing severe hitching when turning to face a different direction, plus 1% lows of 20-30fps on a modern fairly high-end system so it might be 60fps but it's dropped frames constantly and 40-100ms latency... it feels like my mouse is moving through custard.

It seems to be when I'm running the game at native resolution 3840 x 2160p (4k) which I understand is high but as the game is old it shouldn't be a problem at all for a modern system.
My GPU is still barely using 20-30% and just a few GBs of VRAM plus the CPU is hardly even above idling as well, so my system is barely being used and yet it's like the game has maxed out the resources. I did check that the 4GB patcher was running using the console method (GetItLCC) and it says it's working.

I tried dropping AA to off which helped a little, I tried dropping from ultra to high which also helped a little, and I tried running it at 2560 x 1440 which helped a lot but then it looks blurry as it's not scaling well because of the screen resolution.

So I'm really confused and also concerned if I start adding some texture packs and maybe an ENB or lighting tweaks... Is the game even going to be playable!!
Are there any known issues with modern hardware such as having to use really older drivers or compatibility tweaks? I've experienced this with other games that newer drivers would actually make performance worse in old games.
Or is it because the game is very old does it just generally have a problem running at 2160p?

I would really appreciate some feedback and advice, thank you so much.

I cant imagine why running the game at 3840 x 2160p (4k) would lagging as hell.

I mean when the game was out that kind of resolution isnot even exist. So yeah, i dont know why running at that res would be problematic. Oh no.

[Fixed]
I finally discovered what the problem was, I didn't even considered this as I've never once run into this issue before.

My setup as I work and game on the same system (dual boot) has a few 1440p work and daily monitors that are in use all the time, and one larger and much nicer 4K panel that is only for gaming, and because it's OLED I have it completely off unless I'm about to sit down for a gaming section as I don't want any risk of the system turning it on and then it sitting for hours with a static image.

As I was only setting up the game and installing mods, not actually playing it yet, I was using the work monitors that are 1440p and just setting the game rendering resolution to 2160p (downsampling) so it would give me an accurate idea of the FPS when I'm actually playing on the gaming panel, something I've done hundreds of times on different monitors, systems, and games for more than a decade never with any issues except maybe a slightly blurred image if it didn't scale very well.

However, It seems this game has a real issue with downsampling, because if I run the game on the 4K panel it runs at a solid 144fps with 1% lows of around 100 to 120 and latency between 5 and 15ms. Even after a fresh install and running no mods at all the base NV game was running like crap, but last night I decided to try it on the gaming display and suddenly it's running buttery smooth.
I tested it on one of the 1440p monitors again and it ran like horses**t, back to the 4K panel and it's silky smooth.
I wondered if it was the monitors themselves so I changed the rendering resolution in the Nvidia Control Panel so that the monitor was actually rendering at 4K not just the game and then tried running the game again and it ran perfectly, so I set it back to native resolution 1440p and it ran like dog dirt... changed it back to render at 4K and its silky smooth again. TF! 🤔

In all my years of downsampling games having them render at higher resolutions than my monitor to increase the visual/graphics, I've never once run into this issue or even heard of it!

Very very strange, but I now know the cause and the game is running like a charm.

[TL;DR] - Downsampling (rendering at a higher resolution) was the cause for some weird reason.

How is the game still fine when you run it at native resolution 3840 x 2160p? If I do that, the game will be lagged
